Chronic Disease Self Management Program

HIP-Cuyahoga’s Chronic Disease Management subcommittee recently presented its work on the Chronic Disease Self-Management Program at the Better Health Partnership’s Learning Collaborative on April 8^th, and at the Research ShowCASE on April 15th at Case Western Reserve University.
